The Alzheimer’s Society Dementia Navigator Service
Surrey County Council and the Clinical Commissioning Groups commission the Alzheimer’s Society to provide one to one personalised dementia support through its Dementia Navigators. They offer information and practical guidance to help you understand the condition, cope with day-to-day challenges and prepare for the future. You can access support face-to-face, over the phone or in writing to help you.

Crossroads Care Surrey
Surrey’s leading charity providing respite breaks for carers, eases the strain on those who care for loved ones 24/7. In 2019 alone Crossroads Care Surrey helped 2,049 unpaid carers by regularly lending a hand with care to 1,264 loved ones so people who care can have a break and recharge both physically and emotionally.

Surrey & Borders Partnership NHS Foundation Trust (SABP)
Surrey and Borders Partnership NHS Foundation Trust is specialist provider for health services for people of all ages with mental ill-health in Surrey. They deliver a range of specialist mental health support such as Young Onset Dementia services, Intensive Support Team for Dementia, Community Mental Health Team for Older People and much more. Some of their services will require a referral from a GP.

Dementia Services for People with Learning Disabilities
The Community Learning Disabilities Teams offer a comprehensive assessment and support service for people with learning disabilities who are at risk of developing dementia.
We offer baseline assessments for people with Down’s Syndrome aged 30, dementia diagnosis and post diagnosis multi-disciplinary support for people with learning disabilities, their carers and support staff.

COVID-19 remote monitoring support for older people with mental ill-health and their carers
The TIHM Monitoring Service is a new, free NHS service that is remotely monitoring the health of older people with mental ill-health and their carers during Covid-19.
The service, which is currently accepting referrals, is free of charge and available to people of any age diagnosed with dementia or mild cognitive impairment and people aged 65 and over who have a diagnosis of depression and/or anxiety.
